Consumer Health Data Privacy Policy

This policy is provided under Washington's My Health My Data Act (RCW 19.373) by Lyfe Max LLC.
Consumer health data we collect, and why
We don't ask for health information. However, the following may be treated as consumer health data under Washington law:
- Your email address together with your sign-up for Lyfe Max launch updates, because it shows interest in evening light-management eyewear. We use it only to send the product updates you requested.
- Any health information you choose to include when you email us. We use it only to respond to you.

We collect this information directly from you, when you submit the sign-up form or email us.
Consumer health data we share
We share the data described above only with service providers that process it on our behalf to deliver what you requested. We don't sell consumer health data.
Third parties and affiliates we share with
- Website hosting and form-processing provider
- Email service provider
We have no affiliates that receive consumer health data.
Your rights and how to use them
You have the right to:
- confirm whether we collect, share or sell your consumer health data, and access that data;
- get a list of the third parties and affiliates we have shared it with, with their contact information;
- withdraw your consent to our collection and sharing; and
- have your consumer health data deleted.
To make a request, email bing@getlyfemax.com with the subject "Consumer health data request." We may need to confirm that the request comes from you. We'll respond within 45 days, which may be extended once by another 45 days when reasonably necessary; we'll tell you if that happens.
If we decline your request, you can appeal by replying with the subject "Appeal." We'll give you a written decision within 45 days. If your appeal is denied, you can contact the Washington State Attorney General at atg.wa.gov/file-complaint.
